What's Actually Causing an Ingrown Toenail?

Here's something that surprises most patients: in many cases, the nail isn't actually growing into the skin. What's really happening is that the soft tissue at the edge of your toe becomes swollen and inflamed, pressing up against the nail. The nail stays put — the tissue moves toward it.


That said, the nail's shape and how it's been trimmed absolutely plays a role. Causes include:

  • Cutting nails too short or rounding the edges — this encourages skin to grow over the nail border

  • Tight shoes that squeeze the toes and increase pressure on the nail borders

  • Trauma or injury to the toe that causes swelling or changes nail growth

  • Genetics — some people naturally have curved or wide nails that are more prone to this problem

  • Poor foot hygiene that allows moisture and bacteria to irritate the nail border

For cases without infection, we can often resolve the problem in a single visit. The nail edge is trimmed at an angle to remove the pressure on the swollen tissue. Recovery is 1–2 weeks and most patients return to normal shoes the same day.


Nail Avulsion — Removing the Problem Nail Edge

When the ingrown edge needs to be fully removed, we perform a nail avulsion — carefully lifting and removing the affected portion of the nail under local anesthesia. This is a simple in-office procedure with a 2–4 week healing time. The nail grows back normally in most cases.


Matrixectomy — The Permanent Solution

If your ingrown toenail keeps coming back, a matrixectomy is the answer. After removing the problematic nail edge, we use a chemical (phenol) to permanently destroy the section of the nail matrix that keeps regenerating the ingrown edge. This has an excellent success rate and ends the cycle of recurrence for good.


Recovery takes 3–6 weeks, but the result is permanent relief from that side of the nail. It's the most effective long-term solution we have, and it's performed right here in our Cypress office.


What If My Toe Is Already Infected?

If there's active infection present, we treat that first before any nail procedure. Performing surgery on an infected toe increases complication risk. We'll prescribe appropriate antibiotics and wound care, then schedule your procedure once things have calmed down.


Signs of infection: redness spreading beyond the nail border, warmth, pus or drainage, increasing pain, or fever. Don't wait on these — call us same day.


Post-Procedure Care Tips

  • Keep the foot elevated for the first 24–48 hours to minimize swelling

  • Soak the foot daily in warm water with Epsom salts

  • Change dressings regularly and keep the area clean and dry

  • Wear open-toed shoes or sandals until healing is complete
